The process begins by completing my various forms, followed by an appointment to discuss your health concerns. Please bring all copies of lab results from the past few years  It saves time if you have typed up a chronology of your life from birth with all significant illnesses, surgeries, major stressors and symptoms. Our initial face to face appointment will allow us to review your past records together to ensure clarity, and to go over the information you have provided on the questionnaires. This review will give me a detailed comprehension of your body's particular needs.

In order to effectively provide treatment I ask that you complete the Nutritional Assessment Questionnaire provided by the Nutritional Therapy Associates.  I use the results of the questionnaire to complete a computer analysis to ensure all pertinent symptoms have been thoroughly assessed. This form should be mailed to us at least 2 days before your first appointment, if possible.  Please use numbers all the way through the form.  You can leave zeros blank.

Additional testing may be required to determine precisely what is troubling you. This usually entails making an appointment for the testing, then sending samples to a laboratory for analysis. For added convenience, I can help you manage your health remotely by using laboratories that are local to you. The collected information will reveal to me how your body is responding to treatment.

Please understand that I do not function as your primary care practitioner. Although I am fully qualified to perform primary care, I do not believe it takes full advantage of my unique skills and abilities. My strength lies in caring for patients with chronic complaints that have been written off by other medical practitioners. My patients find that I take them seriously. I will continue my investigation until I can discover what is happening with your health. It may help to think of me as an expert consultant for chronic health problems, rather than the provider that does your yearly physical.

My consultation fees are based on how much time is required to thoroughly review your health history and interview you, to identify precisely what I can do for you. Payment is due at the time of service. We do not bill insurance, but we do provide two copies of a receipt complete with all codes necessary for patients to submit to their own private insurance companies for reimbursement. Please note that I have opted out of Medicare and Medicaid, so none of my services are reimbursable through those plans. If you are eligible for Medicare services and want to become a patient, you will need to fill out the Medicare Contract form.
